If you have any questions regarding the Healthy at Home Eviction Relief Fund, please review the frequently asked questions (FAQs).
Applicant Type

Eligible Tenant applicants must meet ALL of the following:
Are you a tenant in any Kentucky County outside of Jefferson County?
Are you at risk of eviction as evidenced by a pending court eviction or a past due notice from landlord?
Is your household income (based on the past 30 days) < 80% of Area Median Income (AMI)? Click Here to see the monthly income eligibility limits for your county by family size.
Do you have a current lease or can you execute a lease/addendum?
Can you certify that you DO NOT currently receive other rental assistance? The program cannot duplicate assistance from another source, including Section 8, USDA RD rental assistance, and public housing?
Are you requesting assistance for your primary residence?
Required documentation from TENANTS:
(You must upload electronic versions or photos of these documents to complete your application)
  • Copy of state ID OR mail indicating proof of residence.
  • Proof of all income earned by all household members over the past month or 4 weeks.
  • Current lease in tenant’s name (if a written lease was completed).
  • Letter/notice of late rent, notice to vacate, court summons, judgement, etc.
Eligible Landlord applicants must meet ALL of the following:
Will you forgive up to 10% of rent due and forgive late fees, penalties, and interest related to non-payment of rent?
Do you agree to give 45 days' notice for any future eviction, not to be initiated until at least 30 days after assistance concludes?
Do you agree to terms of arrearage settlement and to submit all necessary paperwork? Click Here to download the document.
Required documentation from LANDLORDS:

  • Copy of owner's state ID.
  • Current Lease in landlord and tenant’s names (if a written lease was completed). If no lease exists, one must be executed and submitted at a later date if you and tenant are approved for assistance. Click Here to download the document.
  • Letter of eviction, notice of late rent, notice to vacate, court summons, judgement, etc.
Documentation required at a later time from LANDLORD if approved for assistance:
  • Newly executed written lease if landlord and tenant did not already have one.
